Montverde Academy Athletics

The Florida High School Athletic Association (FHSAA) requires all student-athletes to have an annual EL 2 Pre-Participation Physical completed by a healthcare provider licensed in the USA. MVA’s Athletic Department offers two on-campus sports physical nights (August & May) in partnership with Florida Sports Injury and Orthopedic Institute to help our families satisfy this requirement. Preregister Today!

If you miss one of our hosted events, Florida Sports Injury and Orthopedic Institute also accept walk-ins at their office, Monday - Friday between 10am - 3pm. Please contact them directly for additional information regarding their office hours and the services they offer.

Additional Information:

• On-campus physicals will take place in August and May. We will email families with details once dates are confirmed.
• Doors will open at 3:30 pm and close at 5:30 pm.
• Day students MUST have a parent/guardian accompany them in order for the physician to perform their evaluation.
• Register using the link above or walk-in the day of the event!
• The FHSAA EL2 Physical form will be provided on site for every registrant.
