Magnus Health Student-Athlete Registration

Varsity, JV and Academy Programs

All students interested in participating in a Varsity, Junior Varsity or Academy (CBD, SIMA, MAT) program must be compliant in Magnus Health prior to any team-related conditioning, try-out, practice, or competition. Login to your Veracross My MVA Parent Portal and navigate to Magnus Health using below as reference:

A set of conditional questions will be listed for you to answer accurately, so that the appropriate required forms populate for your child. All requirement forms must be submitted and then reviewed/approved by our infirmary and athletic admin team before clearance of activities.

This registration process is intended to ensure compliance with Montverde Academy Athletic Department policies and Florida High School Athletic Association (FHSAA) requirements.

Athletic Eligibility Forms & Tips

  1. EL 1 – Electrocardiogram (ECG) Heart Screening | Orlando Health CareSpot ECG Scheduling
    • Required for all 9th graders and student-athletes that are new to high school sports in Florida
      • ECGs completed after July 1st, 2024, can be accepted if completed by a USA licensed healthcare provider
      • ECGs optional for returning 10th-12th graders & Middle Schoolers playing on Varsity, JV or Academy team
  2. EL 2 – Pre-Participation Physical | MVA On-Campus EL2 Physical Registration
    • This EL 2 physical form can only be accepted if completed by a healthcare provider licensed in the USA
    • Ensure entire form is complete with guardian, student and healthcare provider information, signatures & dates
  3. EL 3 – Consent and Release from Liability Certificates
    • School District: Lake County | Guardian(s) and Student must sign/date each page
    • Line-item A, on Page 1 of 5, only list sports that your child is restricted from participating in
    • Line-item G, on Page 1 of 5, please select/provide medical insurance information
  4. GA 4 – Affidavit on Athletic Recruiting & Non-Traditional Student
    • Only required for new transfer 10th - 12th grade student-athletes enrolling into MVA
  5. National Federation of High School (NFHS) Course Certificates
    • All student-athletes must complete the NFHS courses listed below annually. The student's full name must be displayed on each certificate of completion.

1. Concussion for Students 
2. Heat Illness Prevention 
3. Sudden Cardiac Arrest 
4. Sportsmanship
